2. Access to areas with login and password are restricted to people previously registered. Each authorized user is responsible for their data (login and password) and data security.
3. Your access data (login and password) are personal and non-transferable, as well as the responsibility of the service provider user (Health Professional) and the service requesting user. The EKG REPORT is not responsible for innapropriate use of the system.
4. The healthcare provider user is responsible for the veracity of the information provided at the registration, such as full name, specialty and registration number in the class council, all in accordance with the respective ethics code of the area. The user is also responsible for preparing the electrocardiogram (EKG) test report and sending it to the requester. The requesting user is responsible for the veracity of the patient’s additional data in which the EKG was performed, informed at the moment of sending the exam for the report, as well as for the quality of the image sent, and must follow the recommendations to achieve a high graphic quality. The EKG REPORT team may request for the repetition of the exam when considered of low quality.
5. EKG REPORT ensures the security and confidentiality of the data sent. The exam will be kept store in our files indefinitely.
6. EKG Report cannot be held responsible, in any case, for any direct, indirect, incidental and special damages related to or derived from this website and app or its use, or any linked website, resource or app, information referenced or accessed through this website, or by using or downloading, or accessing, any materials, information, products and services, including, without limitation, any lost profits, business interruption, lost savings, loss of programs or other data. This exclusion and disclaimer applies to all causes of action.
7. The charge will be made for each unit exam submitted after the preparation and submission of the report, including the reports in which poor graphic quality is verified, suggesting the repetition of the exam. The charge will be made following the pre-specified values on the website on the date of submission through the applicant's credit card used at the moment of registration.
8. Payment to the healthcare provider user will be made via electronic transfer to the current account informed at the time of registration, until the 5th day of the following month, following the amount informed at the time of registration.
9. After submitting the exam, the report will be available within a maximum of 24 hours from the time of receipt.
10. EKG REPORT reserves the right to change the terms and conditions, as well as the content of the website, at any time, without prior notice.
11. Users declare that they are fully aware that their registration at EKG REPORT does not create an employment, corporate and/or any other relationship of any nature with it.
12. The user declares that is aware that EKG REPORT may request, at any time, specific information regarding his registration, in view of the security policy adopted, in order to protect the safety of users and customers.
13. The innapropriate use of the website and application, for example, the provision of incorrect and/or untrue data or any other use that goes against the purposes of EKG REPORT, as well as its privacy and security policies, guarantees its right to terminate and/or prohibit, at its discretion and without prior consultation, user's access.
14. The requesting user has the right to use the service indefinitely, and may, at any time, stop using the service, without prior notice, under no penalty.
15. The conditions established in this term are in accordance with Brazilian legislation and, in agreement with the same, the user submits to the Central Forum of the District of São Paulo, with express waiver of any other, however privileged it may be, tosettle any situations related to its use.
